{"id":123,"date":"2016-04-06T14:41:54","date_gmt":"2016-04-06T14:41:54","guid":{"rendered":"https:\/\/wellesleyps.org\/faculty\/?page_id=123"},"modified":"2025-12-18T13:22:05","modified_gmt":"2025-12-18T18:22:05","slug":"field-trip-approval-checklist","status":"publish","type":"page","link":"https:\/\/wellesleyps.org\/faculty\/field-trip-approval-checklist\/","title":{"rendered":"Field Trip Approval Forms &amp; Checklist"},"content":{"rendered":"\n<h4 class=\"wp-block-heading\">Field Trip Approval Checklist<\/h4>\n\n\n\n<p>Before submitting your Field Trip Approval Form, did you remember to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Read the <a href=\"https:\/\/policies.sparqdata.com\/masc\/browse\/wellesleyset\/wellesley\/IJOA\">Students Field Trip Policy IJOA<\/a>\u00a0to ensure your trip meets the WPS criteria and all requirements have been met.<\/li>\n\n\n\n<li>Review the field trip with nursing staff to ensure trip protocol addresses health\/medical needs of students, whether a nurse is need on the trip, or other medical care needs<\/li>\n\n\n\n<li>Obtain all necessary documentation from drivers if private vehicles have been approved for the trip, including:\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/wellesleyps.org\/faculty\/wp-content\/uploads\/sites\/5\/2016\/04\/fy15_field_trip_driver_approval_form.pdf\">Driver Verification Form<\/a>,<\/li>\n\n\n\n<li><a href=\"https:\/\/wellesleyps.org\/faculty\/wp-content\/uploads\/sites\/5\/2016\/04\/Consent-Form-REVISED-9-1-17.pdf\">Field Trip Consent Form<\/a> and<\/li>\n\n\n\n<li><a href=\"https:\/\/wellesleyps.org\/faculty\/wp-content\/uploads\/sites\/5\/2016\/04\/cori_form_rev_08-31-2015.pdf\">CORI<\/a> (if not already on file), for each driver\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/wellesleyps.org\/faculty\/wp-content\/uploads\/sites\/5\/2016\/04\/CORI-Form-Rev-08-31-2015_SPA-23-Sept-15-MAPA.pdf\">Formulario De Solicitud De CORI<\/a>\u00a0(CORI Form in Spanish)<\/li>\n\n\n\n<li><a href=\"https:\/\/wellesleyps.org\/faculty\/wp-content\/uploads\/sites\/5\/2016\/04\/CORI-Form-Rev-08-31-2015-Simp-Chin-23-Sept-15-MAPA.pdf\">CORI \u8bf7\u6c42\u4e66<\/a> (CORI Form in Simplified Chinese)<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Review the <a href=\"https:\/\/wellesleyps.org\/faculty\/wp-content\/uploads\/sites\/5\/2016\/04\/Field-Trip-Requirements-revised-06-22-17.pdf\">Approval Criteria grid<\/a> in the policy to ensure you are aware of which approvals are necessary, and then follow up to ensure form is on track for appropriate approvals.<\/li>\n<\/ul>\n\n\n\n<p>Approval forms require multiple signatures.
